The ALLIANCE is excited to be recruiting for new members of our Community Links Practitioner team. We are looking for full time and part time Community Links Practitioners that will be based in GP practices in Glasgow.
You will provide a person-centred service that is responsive to the needs and interests of a GP practice population. You will support people to identify issues that affect their ability to live well. The post-holder will work collaboratively with individuals to support them through decision making processes and help them decide upon the issues that they would like to address; helping them overcome any barriers to addressing these.
You must have experience of working with people who are experiencing complex social and emotional circumstances, and a strong understanding of the challenges faced by people living in areas of socio-economic deprivation in relation to living well.
