Night Care Team Worker

We are looking for a passionate individual to work within the Care Team in our Glasgow Residential. You will be responsible for the evening and nights in the therapeutic programme, ensuring an effective and high-quality programme is delivered.

You will be responsible with co-ordinating residents' tasks and activities, administering medication, H&S checks, and providing waking night cover, ensuring that risk is monitored and minimised. You will assist the residents to prepare for their daily programme and activities.

As a Night Care Worker, you will support individuals at night in the residential rehabilitation programme to achieve outcomes that help create long-term recovery. Using the Therapeutic Community “community as method” model you will empower individuals, families and communities to be confident about recovery.

You will support the evening running of the therapeutic community, ensuring residents needs are met and the best possible care is provided in an environment that promotes a culture of recovery in line with a recovery orientated system of care and the Quality Principles.

Whilst also being confident to deliver activities and specialised interventions in accordance with the TC manual and the needs of the service.

This is a full-time role working 37.5 hours per week, on a rota basis. Shifts are expected to be 8pm to 8.30am and consist of 3 shifts per week.

About You

  • Relevant experience of working in health and social care, ideally within the field of substance use, mental health or another comparable field

  • Evidence excellent organisational skills
  • To be proficient in the use of Microsoft Outlook and Word and be familiar with database management systems
  • Excellent communication skills, both written and verbal
  • Understand complex issues presented by people entering abstinence based residential settings
  • Determined, with a drive to succeed and a willingness to learn
  • Passionate and enthusiastic about making a real difference to the lives of people we support

The support we provide is always person centred, we do this by involving people who know the person well, this includes family and friends, other health and Social Work professionals, Advocacy services and our own staff members.

The Stroke Nurse plays a key role in supporting a seamless transition from hospital back into the community for people who have experienced a stroke and their families. The role focuses on helping individuals adjust to life after stroke

by providing personalised information, advice and emotional support. A core part of the role is enabling people to develop confidence in self management, supporting them to build the skills and knowledge needed to manage their

condition, maxinmise independence and improve their long term wellbeing.

The Stroke Nurse will also help deliver the evidence-based, six-week Chronic Disease Self-Management Programme, supporting people to build confidence and skills in managing their long-term health.

We are seeking an enthusiastic individual who is organised and motivated, with good communication skills. Candidates must have a first level general nursing qualification and have a minimum of 2 years’ post-registration experience in

either a hospital or community setting. Experience working with people affected by stroke is essential.

Support may include, confidence building, identifying self-management techniques, access and signposting to other support, goal setting and planning. We are seeking an experienced Support and Development Worker, confident in delivering support to people who have challenges around trauma, recovery and mental health.

Regular administrative tasks are required for this role, such as completing case notes, risk assessments and outcome measures. This will include maintaining records in accordance with data protection and use of Microsoft and other IT programs.

You will maintain positive and effective working relationships with a range of stakeholders, key partners, and relevant professionals. Attendance at the Scottish Borders Mental Health Information Stations are also a part of this role, supporting people in the community to access advice and support around their mental health and wellbeing.

If you have experience of supporting people to improve their mental health and wellbeing, or personal experience of managing your own mental health, we’d love to hear from you!

This is a community outreach post and travel is required across the Scottish Borders as part of the role. A good understanding of the locality and community is desirable, applicants with a driving licence and access to a car are preferred.
