Purpose of the service

The purpose of a homeless youth accommodation service is to provide safe, stable, and supportive housing for young individuals experiencing homelessness. This type of service aims to address the unique challenges faced by homeless youth and offer a range of support to help them transition to stable and independent living.

Purpose of the Role

The Practitioner for Youth Services plays a crucial role in delivering practical and emotional support to empower young people through a strength-based trauma informed approach, enabling them to make informed choices about their lives. The service aims to evidence and monitor the progress of young individuals through regular assessments, ensuring their safety and well-being. Additionally, the role involves supporting young people in various aspects, such as housing benefit applications, understanding housing options, accessing education and employment opportunities, and connecting with community resources. The service is dedicated to delivering a supportive environment and seeking feedback from young people to continuously improve the quality of the service.

Role

As a Support Worker, you will be an integral part of a dedicated team providing safe, secure, and supportive accommodation for those who use our services. Your role will involve offering both practical and emotional support, empowering individuals to build confidence, develop essential life skills, and achieve their personal goals in all aspects of daily living through encouraging independent living.

Main Responsibilities:

The ARCH Resettlement Centre is a dedicated accommodation facility for vulnerable, homeless men aged 21+, providing tailored support to help them navigate the transition to independent living. As a Resettlement Support Worker within this service, you will play a vital role in delivering person-centred care, working as part of an established team to empower vulnerable men in a safe and supportive environment. This role operates on a 4-week rolling rota, which includes a variety of shift patterns, including evenings, weekends, and public holidays, ensuring continuous support for the individuals in our care. This role is on average 40 hours per week.

The Support Worker will:

• Collaborate with the team to provide safe, secure, supported accommodation for homeless individuals.
• Utilise the case management system to support, review, monitor, and evaluate, ensuring positive, person-centred outcomes in compliance with organisational, statutory, and legislative requirements.
• Maintain and update all recording systems and documentation, including occupancy agreements, in accordance with relevant policies, procedures, and practice guidelines within required timeframes.
• Plan and facilitate support group work, meetings, reviews, and other internal/external meetings in alignment with the individual's support plan.
• Offer general advocacy and advice, assisting individuals in meeting the conditions of their occupancy agreements.
