About the role

We’re hiring a SDS Link Practitioner to join our busy brokerage team. Working alongside colleagues in this unique role, you will provide the people we support with the opportunity to exercise choice and control over their care needs and how this is delivered.

Self-Directed Support enables people with the choice to receive ‘personalised’ care that reflects their needs and wishes.

At Capability Scotland we provide independent advice and support and help to navigate the social care map in Dumfries & Galloway, from pre-assessment support, outcome planning and developing peer support connections, we aim to support people in their journey. Our aim is to support individuals in choosing the right SDS option for them and support them to achieve their desired outcome through partnership working with the multidisciplinary team.

Often the people we support are coming to us in a time of crisis, meaning you have to be empathetic, understanding and willing to go that extra mile to help them navigate the care system and obtain the support they need.

Managing a caseload of around 30 people we support, typical daily duties include:

  • Providing information, advice and support on SDS and all the four options.
  • Liaising with existing and new people that we support to establish their care needs, including, collating care plan needs/ outcome plans.
  • Drafting recruitment adverts and linking with HR services to advertise and generate interest.
  • Guiding the people we support to undertake safer recruitment of personal assistants and become good employers
  • Assisting the people we support to effectively manage their personal budgets
  • Working alongside social work to navigate the intricacies of managing an SDS award
  • Supporting people with accessing payroll, insurance and indemnity cover
  • Providing informal advocacy for the people we support
  • Being present within the local community to promote drop-in support and to facilitate peer support sessions
  • General administration (file management, call handling & data recording and reporting)

What you need to know

  • Hours: 35 hours per week, Monday - Friday. (We can offer hybrid and flexi-working).
  • Location: Compass House, Afton Drive, Dumfries, DG2 9ES (with travel)
  • Closing date: 28th August 2024 @ 12pm with Interviews being held 29th August 2024.
  • Due to the travel requirements of the role, a driving license and access to a vehicle are essential.
  • Applicants should have a minimum SVQ3 in Health and Social Care or be willing to work towards this.
