Role

The ALLIANCE is excited to be recruiting for new members of our Community Links Practitioner team. We are looking for full-time Community Links Practitioners that will be based in GP practices in West Dunbartonshire.

You will provide a person-centred service that is responsive to the needs and interests of a GP practice population. You will support people to identify issues that affect their ability to live well. The post-holder will work collaboratively with individuals to support them through decision making processes and help them decide upon the issues that they would like to address; helping them overcome any barriers to addressing these.

You must have experience of working with people who are experiencing complex social and emotional circumstances, and a strong understanding of the challenges faced by people living in areas of socio-economic deprivation in relation to living well. Good administration skills are essential to this role.
