The Support Worker will provide a combination of personal care, care at home and housing support to people with dementia in their own homes. The post holder will be part of a dedicated team that will ensure that the highest quality of service is provided.

The Support Worker will work 2 or 3 shifts per week with the flexibility to increase hours to meet the demands of the service. The Support Worker will work day shift, backshift, weekends and nightshift.

We are looking for a unique person who can support people with dementia, living within their own home in a Supported Living Service, personalised to the tenants needs.

Our support workers are our most important people as they provide front line support.

Our support workers help tenants to maintain skills and independence by providing support and care with all aspects of daily living

Successful applicants will have a positive approach to dementia. You will also have good communication skills and a caring attitude. A willingness to learn and participate in training is essential. Relevant qualifications and/or experience of working with people with dementia is essential.

Here at BTA, we have been given the fantastic opportunity of representing SAMH in their search for their new Corporate Partnerships Manager. Your expertise in account and relationship management will be crucial in building long-term, sustainable financial support where you will play a pivotal part in managing corporate partnerships that provide vital funding for SAMH mental health programs.

In this role you will be responsible for managing multi-year partnerships and delivering exceptional stewardship journeys for corporate supporters. Additionally, you will have a vital role in the Account Management team, while also collaborating closely with colleagues across the Fundraising Department and wider SAMH to fully maximise all opportunities for our partners.

Key responsibilities include:

• Direct Line Management – Provide leadership and guidance to a team of two Account Managers, supporting their development and success. You will also support the Senior Manager in maximising the corporate income stream through partnership work.
• Account Management – Understanding funding mechanisms and processes is required to create relevant and innovative offerings tailored to partner needs making SAMH more attractive to corporate sponsors.
• Maximising Partnerships - Delivering high-quality, impactful partnerships that consistently meet or exceed partner expectations. Executing partnership projects flawlessly, building trust and paving the way for future collaboration and increased financial contributions.
• Relationship Management – Maintaining the current partnerships within SAMH, building and growing ongoing relationships to encourage long-term support and funding.
• Representation – Act as a passionate ambassador for SAMH, effectively representing our values and mission to external stakeholders and partners and advocating for mental health awareness and support in all interactions.

What does SAMH need from you?

Experience:

• Strong experience in meeting and exceeding income or fundraising targets, with the ability to develop and implement successful partnership strategies.
• Knowledge in managing corporate partnerships, ideally within a charity or not-for-profit sector.
• Demonstrable experience of line managing staff, including supporting and developing staff – desirable.

Key Skills and Attributes

• Excellent negotiation, communication, and presentation skills, with the ability to engage diverse stakeholders.
• Strong verbal and written communication skills, including confidence in public speaking/presentations and report writing.

Transform Forth Valley is offering this post in partnership with the Falkirk Health and Social Care Partnership.

This post will be split with 3 days in the Falkirk Self-Neglect and Hoarding Service, and 2 days in the Falkirk Social Inclusion Project.

The aim of the Self-Neglect and Hoarding Service is to support people living in Falkirk who are experiencing issues with hoarding. We currently have an established Self-Neglect and Hoarding Service in Falkirk & Clackmannanshire, with this new post expanding our team. Self-neglect is an extreme lack of self-care, it is sometimes associated with hoarding.

People who experience self-neglect often decline help from others; in many cases they do not feel that they need it or can be overwhelmed by their circumstances and are often socially isolated. This often leads to crisis interventions following a significant deterioration. We work with people who are affected by hoarding disorder and other conditions that result in extreme clutter and chronic disorganisation to make long-term sustainable change. This service aims to provide early, effective, and supportive interventions to those adults who are experiencing self-neglect and hoarding.

We aim to provide early intervention to identify need to offer support and improve outcomes, by reducing health inequalities and enabling adults to realise their potential and be connected to resilient and compassionate communities

The Social Inclusion Project (SIP) supports people who use alcohol and drugs and have backgrounds of offending or other particularly challenging behaviour, to access existing services, referred by partner organisations. The SIP model complements the work of existing agencies/services by supporting people to interact with services when their complex needs (such as substance use, homelessness, mental health illness, learning disability, welfare benefits issues, social isolation and / or family dysfunction) present barriers for their access to care. The collaborative approach between the SIP and existing services is proven to deliver an effective means by which people can access services which they might otherwise struggle to interact with.

This post holder will work directly with the service user to enable their access to an integrated pathway across universal services including the ‘Third Sector’ and wider Health & Social Care services. The service aims to tackle the issues associated with social exclusion by supporting the people affected to enable their engagement with community-based supports.

The post holder will report to the Team Leader. They will work in partnership with colleagues from Falkirk Council’s Housing and Social Work, Police Scotland, Scottish Fire and Rescue Service, NHS Forth Valley and Substance Use Services. In partnership we aim to provide services that are specifically designed to identify, engage, and support individuals. This includes issues of substance use with mental health, physical health, exclusion, learning disabilities, offending behaviour, self-neglect & hoarding, anti-social behaviour, housing, and social problems.

The post holder as an employee of Transform Forth Valley will have a corporate responsibility to contribute to the overall performance of the organisation to ensure that contractual outputs and targets are achieved. There is a responsibility for the post-holder to demonstrate a commitment to continuous improvements in service delivery for the benefit of the service user, the service, and the partners.

We are seeking a Senior National Family Support Practitioner (Job Share, 0.5 FTE) to lead the development of our national family support services and provide day-to-day management of the Scottish Families national support team, providing appropriate support, personal development review and performance management to ensure the delivery of high-quality services, the safety and well-being of the staff and effective use of resources.

The Senior National Family Support Practitioner will work together with their job share partner to manage and participate in the day-to-day operational delivery of Scottish Families national support services. Scottish Families’ innovative national family support programme ensures a positive and empowering support approach that respects and recognises individuals’ needs, and balances these with the ethos and capacity of the service. Scottish Families national support services are branded and promoted as Helpline; ‘Click & Deliver’ take-home Naloxone; one-to-one Ripple Family Support; My Family, My Rights; and Bereavement Support.

The post is based at Edward House, Glasgow – Scottish Families’ national office. (Note that staff are currently working between home and office). This is a national post which includes some travel across Scotland. Hours worked will be in line with service delivery requirements, including occasional evenings and weekends when required. The post will be part of the Scottish Families’ staff team and be supervised by the Head of Programmes.

Together with their job share partner, the post-holder will line manage the national family support team; effectively manage the workflow within the national family support team, including caseloads, to ensure that family members’ support needs are met appropriately within a timely manner; directly participate in delivering national support, including scheduling and delivering virtual one-to-one structured evidence-based intervention sessions (includes delivering CRAFT; Community Reinforcement & Family Training); Helpline contact handling; and providing initial bereavement assessment and support, facilitating referrals to bereavement counselling where appropriate. The role includes overseeing the ongoing development of these services, as required, and quality assuring the work of the team by a range of means. The role also focuses on strengthening family support practice and resource development, advancing specialist skills and expertise, developing consistency of approach, and promoting information-sharing and peer support between team members.

We are seeking applicants who are educated to degree level or equivalent, with experience of direct client work, direct work with families, relationship-based practice, and group work. Applicants should have experience in identifying and managing risk; working in partnership; and the development of services; as well as knowledge of issues affecting families affected by substance use and bereavement. We are seeking an excellent listener, with the ability to support individuals from diverse demographics, and to identify and evaluate risk.
