Capability Scotland is a well-established and respected provider of community-based facilitation services throughout Dumfries and Galloway. In the past three years we have grown from a small but very well established and respected service to that of a major contributor to the provision of care and support across Dumfries and the wider region. We currently have a vacancy for an SDS Link Practitioner within our brokerage service.

About the role

This is an exciting opportunity to join our busy SDS brokerage team as an SDS Link Practitioner. Following confirmation of funding from The Scottish Government, Support in the Right Direction initiative we are looking forward to further developing our service to deliver our agreed self-directed support outcomes.

Our service will focus on:

• Providing self-directed support information and advice through a variety of approaches.
• Providing accessible information around SDS rights and legislation.
• Providing practical and informed support around SDS outcome planning, at the earliest opportunity.
• Providing informal advocacy.
• Supporting people to get the most out of their care.
• Championing SDS within the local community and empowering individuals to make informed decisions around their care needs.
• Providing practical support around becoming an employer to personal assistants.

We are looking for someone who is keen to promote the benefits of Self-Directed Support, who will implement national policy into practice at a local level, while maintaining a positive environment and high-quality service. Your role, as Employment Coordinator is to provide support to young people who have barriers to work, to make progress towards and achieve well paid, sustainable employment. You will work in the local community engaging with young people face to face on a one to one and/or group basis to profile their skills and aspirations for work.

We’re on the lookout for a Service Transformation Manager to join us on a 3-year fixed-term basis in Dumbarton.

The Role

As our Service Transformation Manager, you’ll take the lead in developing and transforming one of our biggest services. Our service provides support to eight individuals with complex support needs who all live in their own flats in the same supported living complex.

Working alongside our Branch Leader, Culture Lead, PBS Lead and Depute Delivery Director, you’ll develop a high functioning, accountable and professional support team who are dedicated to making sure that the people we support live the best lives possible.

You’ll take on Registered Manager responsibilities and will drive improvements in standards and quality and help us achieve our strategic aims.

About you

This role would be ideal for someone who is looking to develop their career in our sector in a more senior role, or a Registered Mental Health/Learning Disabilities Nurse with leadership experience.

What we need you to bring:-

• Extensive experience in a leadership role in the Health or Social Care setting
• Significant experience of managing and leading teams, with evidence of improving performance
• Experience of working with individuals with learning disabilities and Autism
• Knowledge of legislation and governance within the Social Care sector, specifically SSSC codes of practice and Health & Social Care Standards
• A sound understanding of professional boundaries, accountabilities, and responsibilities, particularly in relation to working with vulnerable people
• A proven solutions focused approach to problem solving with a flexible approach to emerging or changing circumstances
• SCQF level 9/10 in Social Services and Healthcare or equivalent
