You will be part of Scotland’s leading charity providing support to people with chest, heart and stroke conditions to live life to the full again. Our Hospital to Home services form a nationwide network of local support groups, nurses, volunteers and one-to-one support teams helping families adjust to life with a heart or lung condition or after a stroke.
The postholder will have the responsibility to lead the delivery and ongoing development of CHSS community based support services with a focus on enabling people with our conditions to live with and beyond their new reality. Core to this will be the requirement of the postholder to be a visible and vocal advocate for the charity that ensures our position as the key 3rd sector partner within geographic systems of health and social care. The active and continuous development of senior system relationships will underpin the delivery of the role and ensure the long term success of the charity and the delivery of our No Life Half Lived Strategy.
