Chest Heart & Stroke Scotland (CHSS) is a Scottish health charity, caring for and supporting people after stroke, or diagnosis of a chest or heart condition. We offer vital advice and information to people and their families, delivering support services in the community. We believe everyone has the right to live life to the full, so we are committed to influencing public policy to ensure those affected get the services they so badly need. Our ambition is to be Scotland’s leading organisation for community driven support for people with these health conditions.

You will be part of Scotland’s leading charity providing support to people with chest, heart, stroke and Long Covid conditions, to live life to the full again. Our Community Healthcare Support Service model forms a nationwide network of local support groups, health care professionals, volunteers and one-to-one support teams helping families adjust to life with a heart or lung condition, long covid or after a stroke.

To support the delivery of our ambitious NLHL strategy, CHSS is seeking to appoint a Deputy Head of Service Delivery (Triage, Clinical & Support Line Operations). In this role, the post holder will support the Head of Service Delivery in the operational execution of our clinical services across Scotland and to implement a unified single point of access to CHSS services. This will ensure that routes into our service provision for partners, referrers and those with our conditions, is effective, meets their needs and will enhance the overall value of our offerings.

The post holder will work in collaboration with the Deputy Head of Community Support Services to facilitate a whole system approach to supported self management and will be responsible for leading a team of clinical and operational services staff and volunteers to oversee the operational delivery of the Services Plan with a specific focus on Stroke Nurse and Advice Line service delivery, in line with our No Life Half Lived Strategy.

CHSS is seeking a strong leader who has knowledge of the health and social care landscape and who understands the evolving needs of service delivery and supported self management. Applicants should possess a current NMC or HCPC professional registration and bring strong leadership experience with excellent communication and organizational skills. Confidence in IT skills and experience working to a high professional standard using own initiative is essential.

Our Community Healthcare Support Service provides access to a range of supported self management and community recovery services to support people across Scotland to manage their health as well as possible and adjust to life after a diagnosis of a chest or heart condition or after a stroke.

This is a key role in supporting the Service Delivery team to develop and maintain a high quality and effective insights and evaluation process to support the roll out of the Community Healthcare Support Service Model.

The CHSS Community Healthcare Support service model is a quality supported self-management and community recovery pathway informed by expertise and most importantly, by people with our conditions.

The model includes a range of interventions from clinical support services, structured and evidence based self management workshops, 1:1 community support, health information and more, which are available to everyone with our conditions. Each service is aligned with the principles of supported self management and aims to educate and support individuals to manage their chronic disease.

This model will be evaluated robustly by CHSS within each NHS board in which we work, and we will seek to demonstrate the positive impact that our service delivery can have on patient outcomes, experiences, and on the NHS & social care sector.

They will be the main point of contact for service user evaluation by ensuring that everyone who accesses our services has an opportunity to feedback on their outcomes and experiences in a way that is most accessible to them.

You will be part of Scotland’s leading health charity providing support to people with chest, heart and stroke conditions and Long Covid to live life to the full again. The CHSS Health Information service exists to ensure that people affected by these conditions are supported with information they want, when they need it, and in a way that is appropriate for them.

As the CHSS Health Information Officer you will be responsible for reviewing, developing and maintaining CHSS’s health information in all formats. As well as working on the identified processes involved in maintaining PIFTICK accreditation of these resources.

The post holder will collaborate with the appropriate internal and external professionals and advisory groups ensure that the content of all preexisting and new resources remain current, and evidence based.

As Finance Officer, you will play a key role in supporting the charity’s financial operations, focusing on Purchase Ledger management while contributing to VAT and payroll functions. This is an excellent opportunity for someone looking to develop their finance expertise while working as part of a collaborative and supportive team, helping deliver our No Life Half Lived Strategy.

The Finance Officer role key aims are to manage our purchase ledger financial activities for both the Charity and the Trading Company, support the period end processes and preparation of the monthly management accounts.

We are looking for a detail-oriented and organised individual with experience in finance, particularly purchase ledger management. Strong IT skills, excellent numeracy, and the ability to work both independently and collaboratively are essential.

Our Community Healthcare Support Services form a nationwide network of local support groups, nurses, volunteers and one-to-one support teams helping families adjust to life with a heart or lung condition or after a stroke.

We are looking for a Community Support Coordinator to join our amazing team in the West area. This role is focused on making sure that people affected by chest, heart and stroke conditions get the help they need and get back to doing the things that matter to them.

Through our Community Support Services, you will support people to have the confidence and tools to self-manage their health condition and live as independently as possible. You’ll also help people get back to doing the things that are important to them – from simple things like walking to their local shop following a stroke, to returning to a sky-high hobby of gliding!

You’ll be directly involved in supporting people with chest, heart and stroke conditions through both one-to-one and group services in both community and hospital settings. You’ll work with the rest of the Community Support Services team in Ayrshire, and manage a team of incredible volunteers, to make a real difference to the lives of people with our conditions.

Candidates don’t need to have medical knowledge as training is provided on our health conditions. We are looking for someone with a positive attitude towards people with disabilities and long-term conditions; an understanding of the challenges faced by people with chest, heart and stroke conditions including Long Covid; an understanding of community service provision; the ability to recruit, manage and motivate a team of volunteers; and excellent communication and interpersonal skills.
