Our long-term vision is to end youth homelessness. Our more immediate mission is to ensure that every young person in Scotland has access to expert youth specific services to assist them to avoid, resolve and move on from homelessness, making it rare, brief, and non-recurring.

Context

Working as part of our innovative Fife Housing First for Youth Project, the Project Worker will work directly with young people aged 16-25 years to coordinate and provide emotional and practical support to avoid, resolve and move on from homelessness

Working directly with young people in their home or within a community setting the Project Worker will support young people to secure/maintain a permanent home, improve their independent living skills, access other services, manage finances and engage with resources within their communities.

The Project Worker will offer line management, guidance, and support to Support Assistants and volunteers. The Project Workers are responsible for completing support plans and risk assessments and acting as co-ordinator for any key people involved in the care and support of young people.

About the service

The service works with people who are experiencing challenges such as poor health, social isolation, financial pressures, or difficult life circumstances, supporting them through GP practices in Falkirk East and Falkirk Central. It focuses on early, practical help by offering personalised support conversations that look at the whole person, not just their immediate health concerns.

The aim is to help people feel more confident, supported, and connected within their local community. By working closely with individuals, the service helps them find and access the right local support, build resilience, and take positive steps towards improving their overall wellbeing and quality of life.

About the role

This role works within GP practices to support people to improve their wellbeing through practical, person-centred support and strong links to community services. You will help connect healthcare with local resources so people can access the right support at the right time.

Key aspects of the role include:

• Building trusting relationships to understand people’s needs and goals
• Supporting people to access local services and community-based support
• Working with GP teams and partner organisations to improve joined-up support

About you

We are looking for someone who is confident working with people facing complex challenges, and who can build trust and support positive change in a person-centred way.

Key requirements include:

• Experience supporting people in challenging or complex situations
• Strong communication skills, with the ability to build trusting and supportive relationships
• Ability to work independently and as part of a team, with good organisation and professional boundaries

You’ll be joining a small but experienced and knowledgeable team who will give you full introduction to all aspects of Community Link Service.

You’ll have a supportive manager as well as access to wider Cyrenians support, including our learning and development programme, and staff wellbeing services.

About Us

Street Fit Scotland (SFS) provides health, fitness, and wellbeing programmes for individuals and groups affected by or experiencing homelessness, including people experiencing addiction or affected by alcohol, drugs, and other dependencies.

We deliver a range of physical fitness and wellbeing sessions that focus on improving mental and physical health, reducing social isolation and loneliness, and supporting people to reconnect with mainstream society.

We are seeking a motivated, committed, enthusiastic, organised, and passionate individual to join our team as a Link Worker (Drug and Alcohol).

Purpose of the Role

The Link Worker will provide person-centred support to Street Fit Scotland members experiencing drug, alcohol, and wider health-related challenges. The postholder will work alongside members to increase access to support, strengthen wellbeing, and improve outcomes through a compassionate and harm reduction approach.

The role will involve:

• Providing one-to-one and group support to SFS members experiencing drug, alcohol, and health-related issues.

• Building positive external relationships with partner agencies including housing, addiction, mental health, and wider support services to strengthen wraparound support.

• Managing referrals and signposting pathways.

• Developing support resources and innovative approaches to working alongside members using a person-centred approach.

Role:

This is an opportunity to come and join our team as a Residential Worker primarily based at Cauvin. Cauvin is our small, residential space that supports up to three mums and babies at any one time.

Our team work closely with mums and their babies to provide the advice and guidance they may need to build their attachment and bond with their child, to support their own individual health, wellbeing and independent living skills and ensuring that their babies own growth and developmental needs are being met. We work together with families, social work, health and other colleagues to ensure the strengths and voices of mums and babies are at the heart of all we do while the safety and welfare of the child is always maintained. We work closely with colleagues across the Trust to ensure we are taking a family and community-based approach to work and to create positive onward transitions for mothers and babies.

To support this, our residential role involves a shift pattern over a four-week rota, that requires weekend working, lone working and sleep-ins. This is an exciting and demanding opportunity for someone to be part of our team as we adapt our services to #KeepThePromise. We are looking for someone who is:

• Skilled and experienced in delivering direct work with delivering interventions with parents, babies and families.
• Passionate about working with young people, babies and families.
• Able to build strong and loving relationships.
• Understands child development and trauma-informed practice.
• Is skilled in delivering interventions with parents, babies and families.
• Positive and thoughtful, and rights focused.
