Role

Phoenix Futures is looking for a passionate individual to work within the Therapeutic Team responsible for the implementation, development and evaluation of the therapeutic programme, ensuring an effective and high-quality programme is delivered.

You will manage a client case load, ensuring end-to-end care co-ordination and care plans meet the needs of both service users and stakeholders, and you will be integral in the delivery of the Therapeutic Community programme including the delivery of the group work programme.

As a Therapeutic Worker, you will support individuals accessing the residential rehabilitation programme to achieve outcomes that help create long-term recovery. Using the Therapeutic Community “community as method” model you will empower individuals, families and communities to be confident about recovery.

You will support the day to day running of the therapeutic community, ensuring service user needs are met and the best possible care is provided in an environment that promotes a culture of recovery in line with a recovery orientated system of care and the Quality Principles. Managing a service user case load, providing end-to-end care co-ordination from assessment and admission to disengagement planning ensuring care plans meet the needs of both service users and stakeholders.

Whilst also being confident to deliver therapeutic groups, activities and specialised interventions in accordance with the TC manual and the needs of the service.

This is a full-time role working 37.5 hours per week, on a rota basis including overnights.

About You

  • Relevant experience of working in health and social care, ideally within the field of problematic substance use, mental health or another comparable field
  • Evidence of effective partnership working and the ability to network for the benefit of the client and service
  • Evidence of managing a busy caseload or tasks and excellent organisational skills
  • To be proficient in the use of Microsoft Outlook and Word and be familiar with database management systems
  • Excellent communication skills, both written and verbal
  • Understand complex issues presented by people entering abstinence based residential settings
  • Determined, with a drive to succeed and a willingness to learn
  • Passionate and enthusiastic about making a real difference to the lives of people we support
