The Community Connectors programme provides better access to information for older people. We also support them to access the services they need to live healthier and happier lives with as much independence as possible. The project is delivered in partnership with the Glasgow and West of Scotland Forum of Housing Associations and works alongside local and citywide organisations to strengthen information links between the third sector, older people, carers, and statutory services. The model has been rolled out in the three locality sectors within Glasgow.
We are recruiting for a Senior Health and Wellbeing Practitioner to cover a one-year maternity leave with the benefit of a month-long handover from the existing practitioner. The successful applicant will have access to a wide range of training and the role provides a great opportunity for an existing links/community health practitioner who would like to take on more responsibility and advance their skills in one-to-one, person centred approaches alongside case coordination with a wide range of partners. The post has a high degree of autonomy, responsibility, and decision-making in supporting highly vulnerable individuals and in responding to complex issues. Applicants should demonstrate highly effective communication and negotiation skills with equally effective time and task management abilities. The successful candidate will be supported by the Programme Manager and a team of Client Liaison Officers who provide support with caseload management.
We can offer the option of working from home or from Shettleston Housing Association, the role will largely involve one to one working with older people and/or their carers, in their homes. With the ultimate goal of supporting clients to improve their health and wellbeing and independent living.
