The Role:

The main role of the Community Link Worker is to provide a person-centred service that is responsive to the needs and interests of a GP practice and its patients. This Community Link Worker position is based in the Tollcross Health Centre which encompasses the GP practices of Polwarth, Leven and Gilmore.

The Community Link Worker will support patients referred by the Primary Care teams to:

  • Identify issues affecting their ability to live well
  • Identify personal outcomes and priorities
  • Provide a person centred approach to support patients to overcome barriers to achieving personal goals
  • Link people to local and national services and agencies.

The Community Link Worker will support the GP practice teams to have increased knowledge and understanding of local and national resources available to their patients. They will also build relationships and processes between the GP practice and community resources, statutory organisations, other health services and voluntary organisations.

The post-holder must have experience of working with people who are facing complex social and emotional circumstances. They will also have excellent and effective interpersonal skills, being able to demonstrate an ability to work with people on a 1:1 basis as well as experience in networking and relationship building with other professionals and organisations.

About the Community Link Worker Programme:

The Community Links Worker programme, funded by EVOC and Edinburgh Health & Social Care Partnership, is delivered in partnership with GP Practices and the 3rd sector to support people to live well through strengthening connections between community resources and primary care.

This Community Link Worker post holder will be employed by Space @ The Broomhouse Hub and will be supervised and supported by Space Head of Community & Connections, and the Community Link Worker Network Area Lead for South Edinburgh, who will work together to ensure that the post holder is on track to meet outcomes prescribed by the Scottish Government and Edinburgh Integrated Joint Board.

This post will be based predominantly at Tollcross Health Centre with some possible home working and occasional visits to the Broomhouse Hub.
