Role

Primary Function: To provide the operational development of the Social Prescribing Project, and the day-to-day liaison of related Annexe wellbeing services. Forge working links with external partners such as GPs, and health professionals to create a pathway for patients to access activities and services provided by Annexe Communities and other local community and voluntary organisations.

Condition: The post holder is required to be PVG registered and authorised to undertake regulated work with vulnerable adults.
