We Are With You are offering the exciting opportunity to join our small Community Link Worker team in East Lothian, so take a closer look! We have a 30 hour post available in the Prestonpans GP Practice. You will provide 1:1 and small group support to people referred to you by the Practice team. The role is a ‘generalist’ role; meaning that you will see people facing all kinds of health and wellbeing challenges; including low level mental health difficulties, disability, long term conditions, bereavement, poverty, relationship difficulties, experience of trauma, housing problems, parenting challenges to name just some.
Your remit is to offer evidence-based interventions, to motivate people to make change and to link them with the right tools and support to improve their wellbeing. You’ll build up great connections with the local community, third sector and statutory service; so as to create strong referral pathways for the people referred to you.
