At Beatson Cancer Charity we support and enhance the treatment, care and wellbeing of current, former and future cancer patients and their families. Beatson Cancer Charity’s Living with Uncertainty Service (funded by National Lottery Community Fund) and Bereavement Service (initially funded by the NHS GCC Endowments Fund), support people who are affected by cancer, and their families, to manage the psychological and emotional impact of living with cancer and bereavement. Our team of psychologists and facilitators, work alongside clients to help them improve their understanding of the issues they face and help them live well, in line with their values, and alongside the difficulties they face. This supports them to improve their confidence and enhance their overall resilience.

Role Purpose

As part of a small team, the Facilitator will work under the supervision of a psychologist, to deliver support. This will be delivered within an Acceptance and Commitment Therapy (ACT) and person-centered framework and include, assessment and triage of referrals, delivery of a structured group psycho-educational programme, person centered support and signposting and supported transitions to associated services as appropriate. They will contribute to the development and promotion of the service as a specialist resource across professional and community settings including educational activity across a range of stakeholder groups.

Responsibilities

− Responsible for triage and assessment of new referrals based on accepted psychological frameworks e.g. Psychological Therapies and Support Framework for People Affected by Cancer 2022.

− Undertake risk assessment and risk management for individual clients and refer on as appropriate in response to client need.

− Play an active role in regular referral meetings, including care planning, in order to ensure timely service delivery and support.

− Support the psychologist in the delivery of ACT based group support programmes.

− Provision of level 1 and 2 support/coaching, pre and post group support.

− Communicate appropriate information concerning client’s assessment and on-going support in a skilled and sensitive manner that best meets the needs of clients and referrers.

− Ensure service delivery remains balanced and appropriate through actively managing individuals care and support.

− Liaising and communicating with key stakeholders including delivery of presentations.

− Use TM3 system to maintain up to date records of clinical data e.g. clinical activity, waiting list and outcome measures data.

− Contribute to on-going service evaluation/development via reflective practice, case review, evaluation framework and relevant audit.

Essential Candidate Criteria

− Educated to a degree level or equivalent within a relevant field (Health, Psychology, AHP, Nursing, Social Sciences, Counselling, CBT).

− Experience of providing psychosocial support to people and their families living with a long-term health condition.

− Experience of working as part of a team assessing the needs of individuals and families, planning, and delivering appropriate support.

− Experience of working with people at psychological risk including an understanding of levels of psychological support e.g. CBT, Clinical/Counselling Psychology.

− Experience and strong understanding of validated assessment tools and their use in planning and delivering support.

− Able to travel and be flexible to deliver services/workshops in a range of locations.

The Policy and Campaigns officer supports our Policy and Campaigns manager and the Head of External Engagement to deliver government and parliamentary focused engagement work. You’ll advocate for access to our Community Healthcare Support Service and supported self management, support our Right to Rehab campaign, promote integrated NHS referrals and the role of third sector health organisations in the National Care Service, and work with partners across the public, private and third sectors to ensure there is No Life Half Lived in Scotland following the diagnosis of one of our conditions.

This is a key role in supporting the Service Delivery team to develop and maintain a high quality and effective insights and evaluation process to support the roll out of the Community Healthcare Support Service Model.

The CHSS Community Healthcare Support service model is a quality supported self-management and community recovery pathway informed by expertise and most importantly, by people with our conditions.

The model includes a range of interventions from clinical support services, structured and evidence based self management workshops, 1:1 community support, health information and more, which are available to everyone with our conditions. Each service is aligned with the principles of supported self management and aims to educate and support individuals to manage their chronic disease.

This model will be evaluated robustly by CHSS within each NHS board in which we work, and we will seek to demonstrate the positive impact that our service delivery can have on patient outcomes, experiences, and on the NHS & social care sector.

They will be the main point of contact for service user evaluation by ensuring that everyone who accesses our services has an opportunity to feedback on their outcomes and experiences in a way that is most accessible to them.
