Role
The Role
As the RTN and Community Development Worker you will work alongside the Therapeutic Team responsible for the implementation, development and evaluation of the therapeutic programme, ensuring an effective and high-quality service is delivered.
You will lead the delivery of Recovery through Nature in the Glasgow Residential Service. To take the lead role in delivering, monitoring and reviewing a range of Recovery through Nature (RTN) programmes for residents. Additionally, you will lead on community activities with the planning, risk assessing and measuring outcomes including the impact on social change, addressing inequalities, promoting empowerment and inclusion.
Also, being confident to deliver therapeutic groups, activities and specialised interventions in accordance with the TC manual and the needs of the service.
This is a full-time role working 37.5 hours per week, with occasional weekend work for events and activities, and the needs of the service.

About You
- Relevant experience of working in health and social care, ideally within the field of problematic substance use, mental health or another comparable field
- Evidence of effective partnership working and the ability to network for the benefit of the client and service
- S/NVQ 3 in Care or related field, or a Community Development Qualification or equivalent
- Evidence of managing a busy caseload or tasks and excellent organisational skills
- To be proficient in the use of Microsoft Outlook and Word and be familiar with database management systems
- Excellent communication skills, both written and verbal
- Understand complex issues presented by people entering abstinence based residential settings
- Determined, with a drive to succeed and a willingness to learn
- Passionate and enthusiastic about making a real difference to the lives of people we support

About the job
The Recovery Development Worker is suited to an individual with "lived" and/or “learned” experience of recovery from problematic substance use or addictive behaviours, such as problem gambling, or someone who has been directly affected by another person’s substance use or addictive behaviours.
This experience of recovery and resilience can instil hope and is a powerful example within recovery communities. Individuals with lived experience of substance use must be at least 24 months free from problematic substance use and not engaging with services.
The post holder will be required to join the PVG scheme.

About the Community Link Worker Service
The Community Link Worker service collaborates with selected GP surgeries – supporting adults over the age of 18 through social prescribing. Referrals are received from the GP to our Community Link Workers when there might be a concern caused by social factors, rather than a medical problem. The service focuses on improving health outcomes for people experiencing stressors in their lives due to social problems, which could lead to poor mental health.
About the Role
As a Senior Community Link Worker, you will have a developmental role as well as providing on-going support. You will be responsible for developing positive, working relationships between the Community Link Worker team, Primary Care and local community resources. You will work to ensure that people referred from Primary Care to the Community Link Worker service benefit from community resources which can contribute to improvement of their mental and physical health outcomes. You will contribute to community development in your local area of work by working with community organisations
You will carry case loads for on-going work with specific individuals for developing and reviewing personal support plans, and provide oversight of personal support plans designed and implemented by Community Link Workers. Working closely with Community Link Workers in a guidance and/or support and supervision role, you will promote good working practice and staff development.
You will also ensure that the Community Link Worker service provides an environment where people can improve their health outcomes, establish improved connections in their communities and enhance their quality of life.
Community Link Worker

Purpose of job:
The Community Link Worker service is primarily funded by Edinburgh Health and Social Care Partnership. The principle aim is to support people to live well through strengthening connections between community resources and primary care.
The Community Link Worker (CLW) will be employed by Health All Round (HAR), a community health initiative which aims to improve the health and wellbeing of people living in South West Edinburgh. They will be one of a group of three CLWs who work alongside the wider staff team.
The success of this post is dependent on effective joint work between Edinburgh Health & Social Care Partnership and HAR. The post holder will be supervised by the HAR Deputy CEO with support from the Senior CLW, who will work together to ensure that the post holder is on track to meet outcomes prescribed by the Scottish Government and Edinburgh Integrated Joint Board.
The CLW will provide a person-centred service that is responsive to the needs and interests of Springwell Medical Centre and Murrayfield Medical Practice patient population. They will support patients referred by GPs to identify issues affecting their ability to live well, identify personal outcomes and priorities, provide support to overcome barriers to achieving personal goals and link people to local and national services and agencies.
The CLW will support Springwell Medical Centre and Murrayfield Medical Practice staff and clinicians to have a good, up to date knowledge and understanding of local and national resources available to their patients. They will also build relationships and processes between the GP practice and community resources, statutory organisations, other health services and voluntary organisations.
