Street Connect is a Christian organisation with a mission to offer hope and opportunity of recovery for people
disadvantaged by addiction, homelessness, and poor mental health.
As the Housing & Aftercare Coordinator you can contribute to profound and lasting changes in the lives of
highly vulnerable individuals. In 2025/26, we estimate we benefitted approximately 2,521 individuals across our
projects, with 9,804 street outreach interventions and 5,139 footfalls in our cafés. 9 participants are
currently resident in our supported accommodation. Our model continues to demonstrate consistent progression
across key stages: 511 – around 25% of those reached through street outreach and drop-in cafés went on to
receive more structured support; Of those, 395, or 77% engaged in community recovery groups (315 in 2024/25);
255, or 50% progressed into oneto-one Key Work support (201 in 2024/25); From this group, 35 individuals were
supported to enter into residential rehabilitation (16 in 2024/25). Alongside this, we continue to see positive
outcomes for 28 participants engaged in our Aftercare programme (25 in 2024/25) and 50% of Aftercare
participants were engaged in internships or voluntary placements (14 individuals)
You will work as part of the Street Connect team seeking to develop and deliver community recovery support through a
range of different routes to both male and female service participants who have a background of complex needs
such as addiction, homelessness, and mental health issues, who are now at different stages in their recovery
journey.
This will include both those in aftercare who are living independently and free from such issues and those at an
earlier stage in their recovery.
