MAIN DUTIES AND RESPONSIBILITIES
1. Contribute to ADAPT’s (REACH TEAM) component of the Seek, Keep, Treat Multi-Disciplinary Team (MDT) provision within Hospitals and community locations across Fife, by providing case management, therapeutic psycho-social support and crisis/relapse intervention for patients/clients following medical treatment for alcohol and drug related conditions, accidents and overdoses.
The Multi-Disciplinary Team (MDT) will address the support and treatment needs of patients/clients who are most at risk because of their alcohol and/or drug use but who are least likely to make use of services.
The MDT service is delivered in line with ‘The Quality Principles: Standard Expectations of Care and Support in Drug and Alcohol Services’.
2. Act as a ‘Named Person’ whom patients/clients can rely on for contact and support during their journeys through services by establishing on-going therapeutic relationships that normally begin with contact in hospital and proceed into the community.
3. Provide Case Management for patients/clients ensuring linkage and communication between patients/clients and services. Liaise with Care Co-ordinators, Addictions Nurses, We Are With You Support Workers and specialist Pharmacist within the MDT and with other professionals within Health & Social Care, the Statutory and Third Sectors.
4. Provide Fife ADP Triage Assessment and Referral via FORT to other addiction services within statutory and third sectors. Ensure Triage Assessments are forwarded timeously to enable 90%+ of all Service Users to enter treatment within 21 days of referral date.
5. Provide Client Support with crisis intervention for clients who are living chaotically and/or are at risk of mental and physical trauma, overdose or self-harm and suicide.
6. Provide structured Motivational Interviewing and Psycho-Social therapeutic support for patients/clients who are not willing or ready to attend treatment and rehabilitation services following hospital.
7. Maintain records and provide information for service reporting in accordance with organisational policy and practice.
8. Participate in annual appraisal and regular supervision and be responsible for personal Continuous Professional Development.
9. Carry out any other duties that may reasonably be requested.
