Are you driven to provide a people centric service that breaks through barriers between local and national support services and activities? Can you enable individuals to identify personal outcomes to improve their health and wellbeing? Are you able to develop effective pathways to a range of service providers? The key to this role is in the support of people to live well through strengthening connections between community resources and primary care. This will be born through developing professional working relationships with GP practices and local health improvement teams as well as developing knowledge of other national and local service providers.
This is a fixed term contract until 31 January 2024, the hours are 15 per week which can be discussed at interview. Driver with own car preferred to enable travel between GP Practises.
You will have the ability to build and maintain professional working relationships to enable to the development of effective pathways to a range of service provisions. You will take responsibility for removing barriers to the access of relevant services, and for linking community resources and primary care. You will play a key role in practice team meetings to discuss caseloads, concerns, gaps in service and improvements to be made. You will also implement feedback on learning from the service outcomes and deliver alcohol brief interventions, contributing to Local Drug Partnership targets and HSCP targets.
