You should be pro-active and experienced in using a variety of social media platforms and applications for communication and as an intervention.

You will be able to work independently from GP practices and be very pro-active in finding community resources and helping people access local services and activities to promote their sense of wellbeing. You will have the ability to sustain contact with individuals to assist them to maintain activities. In addition, you should be confident working closely with GPs, practice staff and community agencies. You need to have a high level of IT proficiency, as you are required to record your work daily on a range of IT systems.

You will be required to contribute to a database of local resources, maintain accurate records and actively participate in evaluating, monitoring and reporting this innovative service
