About the Role:

The support we provide is always person-centred, trauma-informed and wellbeing focused. As an Intensive Floating Support Worker, you’ll work in the community to provide a wide range of flexible intensive support to individuals to encourage independence to maintain and sustain a tenancy and adapt to community life.

Main Responsibilities:

This Intensive Floating Support Worker will be covering support to up to 5 semi-independent flats in the community and also proving support to our LaPorte service which is for unaccompanied young people. There will be a requirement to work at Falkirk Hub service where needed. Applicant must be a driver and willing to add business insurance to their policy. This role is working on a rota covering various shift patterns including some weekends. This role involves working 20 hours per week. The Intensive Floating Support Worker will:

  1. Provide intervention and a range of supports for new referrals and existing Supported People in Blue Triangle’s supported accommodation services and move-on accommodation.
  2. To provide trauma-informed, person-centred, and wellbeing-focused support for people supported by Blue Triangle.
  3. To support, review, monitor and evaluate through our SHARP monitoring tool to achieve a positive person-centered outcome for the people we work with in line with Blue Triangle’s statutory and legislative requirements.
  4. To support people transitioning from supported accommodation into the local community or referred from other sources, helping them on their personal pathway towards a settled tenancy, building their tenancy and life management skills.

About the Role:

As a temporary Support Worker, you’ll work as part of a team in providing safe, secure, supported accommodation for the people who use our services. You’ll do this by providing practical and emotional support and encouraging them to achieve their own personal outcomes in all aspects of their daily lives.

Main Responsibilities:

This Support Worker role working at our service in Clydebank involves working on a 4-week rota covering various dayshift and backshift patterns, including some weekend work. This role involves working 35 hours per week. The Support Worker will:

Who we are

Alzheimer Scotland is Scotland’s national dementia charity. Our aim is to make sure nobody faces dementia alone. We provide support and information to people with dementia, their carers and families, we campaign for the rights of people with dementia and fund vital dementia research.

What you’ll do

You will be responsible for supporting people living with dementia to achieve good outcomes using a range of therapeutic interventions, resources and activities as appropriate.

Working as part of the Registered and Commissioned Services/Community Connections team you will have responsibility for ensuring that services are of the highest quality and meet Alzheimer Scotland’s vision for unique therapeutic, highly person-centred support.

You will work with the wider team and Day Services Manager to ensure safe practice and delivery of high-quality, person-centred, therapeutic Registered and Commissioned Services/Community Connections, ensuring practice complies with Alzheimer Scotland’s policies. You will contribute to evidencing, reviewing and continually improving the quality of the services and will participate in internal audit and Care Inspectorate inspections.

You may operate within a building-based Day Care or as part of a Community Connection team delivering Community Groups and Outreach services across an allocated area. At times, this will involve working without supervision in the absence of the Day Services Manager.

You will work with the wider team to evidence that services meet Alzheimer Scotland’s Quality Guarantees, Scotland’s Health and Social Care standards and other quality frameworks including those of the Care Inspectorate where relevant. This includes ensuring that the activities and support offered meet the identified personal outcomes of those attending the services.

What you’ll have

The successful candidate will have a good understanding and/or experience of working with or supporting people living with dementia and their families. This will include an understanding of the benefit of high quality, evidence based therapeutic activity, ongoing evaluation, quality assurance and continuous improvement.

Knowledge and understanding of the Health and Social Care Standards and SSSC codes of practice and the Care Inspectorate and their role in registered services.

Understanding and knowledge of dementia, mild cognitive impairment and Brain Health and experience of working with or supporting people living with dementia and their families.

What you will be doing

You will provide specialist group programmes for women who are recovering from domestic abuse. Maintaining the confidentiality of those accessing the service is vital, as is ensuring that all confidentiality agreements are signed.

You’ll work on your own initiative but liaise closely with team members to ensure all aspects of the service are covered, which requires flexibility to meet the needs of the service.

Promoting effective partnership working and developing strong links with professionals from external agencies and the community are essential to maximise the support available for women. Promoting our work, and the rights and needs of women who experience domestic abuse, to service providers and policy makers is also part of the role.

What you will get

The salary for the role is £14,759.70. You will get 42 days annual leave pro-rata inclusive of public holidays. There’s a pension contribution, enhanced sickness pay and access to a health care cash plan. Full training on the group programme and an extensive induction. The model includes understanding the illness and coming to terms with the diagnosis, peer support, connecting and maintaining links with their community along with planning for future care and decision making to develop an outcome focused plan to support their future hopes, desires, and aspirations.

Supporting our Edinburgh locality, the post will be based within the South East of the city. Alzheimer Scotland’s PDS Link Workers are required to be effective team workers and have the skills to build and maintain relationships with colleagues both within Alzheimer Scotland, the NHS, and other stakeholders.

Skills in using digital platforms are essential as we use a blended approach to support people that involve some virtual meetings and on-line groups.

Applicants must hold a recognised relevant professional qualification, have a good understanding of dementia and how it affects people and their families along with excellent communication skills, a warm flexible approach, combined with skills in empathy and relationship building.

In addition, you should have skills and knowledge at the Enhanced Level of the Promoting Excellence Framework or be able to evidence you are working towards its completion.
