Role Overview
As Dementia Support Coordinator, you will play a central role in coordinating and delivering Town Break’s community-based services, including Town Break at Home and group activities across Forth Valley.
You will act as a key point of contact for people living with dementia and their unpaid carers, coordinating personalised support, facilitating meaningful activities and working closely with partners across health, social care and the third sector.
This is a varied, people-focused role that combines coordination, relationship-building and hands-on support.
About Town Break at Home
Town Break at Home operates Monday to Friday within Forth Valley. The service offers personalised one-to-one support, either at home or out in the community.
Support is shaped around each person’s strengths, interests and goals. This may include social outings, creative activities, gentle physical activity or simply time and space to connect. Frequency of support is agreed through an initial conversation and review process.
