Community Development Worker, Compassionate Communities

Working alongside communities to support people to live well right up to the end of life is a strategic priority for Strathcarron Hospice. To help us further connect people who have long term conditions within their communities, we are looking for an experienced community development worker to help us deliver on our funded community projects and grow our Compassionate Communities work in Falkirk and across Forth Valley.

We are looking for someone with a high level of creativity, responsibility and self-drive who loves working with people and communities. The post holder will build relationships with individuals to find out what skills and gifts people have in order to connect them meaningfully in their own community.

Working alongside our Community Development team your responsibilities will include:

  • Discovering what community support already exists in an individual’s life and community.
  • Connecting people with long term health conditions to their community in a way that is meaningful to them.
  • Recruitment of community volunteers to visit people in their community or accompany individuals to groups in our communities.
  • Working with colleagues in the Compassionate Communities team to support communities to develop the skills, community connection and confidence to enable communities to hold conversations about dying, death and grief across Forth Valley.

Council members provide advice and scrutiny through their participation in these meetings.

An opportunity has arisen as part of a collaborative venture between NHS Forth Valley, Falkirk Health and Social Care Partnership and Strathcarron Community Development

We are looking for an energetic enthusiastic person to join our team of Community Link Workers on a fixed term contract from the 6th Feb 2023 until the 31st March 2024.

The post holder will respond to referrals made by Hospital at Home practitioners. The post holder will make contact with the individual or carer and will carry out a home visit where a good conversation will help to identify what support the person already has in place, and what is available in their local community which might interest them. The post holder will build a knowledge of resources available in the communities and will signpost to these as appropriate.

The successful applicant will need a flexible “can do” attitude, and will need to be able to quickly build relationships of trust with professionals, patients and families.

They will need to be a team worker, but also be able to use their own initiative. An ability to “think outside the box” and find out what really matters to people is essential.

With support from the National Lottery Community Fund we have an exciting opportunity to appoint an experienced Community Builder to work at neighbourhood level in the Fallin community within Forth Valley.

Working alongside key partners i.e. existing services, third sector organisations, community groups and individuals, and using an Asset Based Community Development approach, you will support the building of connected and caring communities that enable people with deteriorating health and their carers to live well right up to the end of life.

We are looking for someone who is confident, inquisitive, resourceful and courageous, driven by the joy of connection. You will be able to spot an opportunity and provide the right conditions for it to develop and flourish. You will be patient and reflective, acknowledging that change happens within communities at the speed of trust. These attributes will enable you to grow community capacity, weave relationships and help local people create positive change and build a compassionate community.

You must have excellent listening skills, be able to communicate with a diverse range of people and thrive on social networking. Ideally you will be educated to HNC level in a relevant field and have a track record of successful asset based community development work.

This role necessitates flexible working (day/ evening/ weekends) and some travel. A car and a driving licence are essential.
