Alzheimer Scotland is Scotland’s national dementia charity. Our aim is to make sure nobody faces dementia alone. We
provide support and information to people with dementia, their carers and families, we campaign for the rights
of people with dementia and fund vital dementia research.

families as their journey transitions from the first point of contact through to end of life. We believe nobody
should face dementia alone.

We are looking for a Community Connexions Outreach Worker for the Fife area. You will be involved in supporting
people living with dementia to establish personal goals and outcomes and help them to achieve these. The
Outreach service is a short-term intervention and support service offering guidance and assistance to people
living with dementia and their carers to help live life to the full and as independently as possible. This may
involve helping to reduce social isolation, building confidence in using public transport, engaging with the
local community and much more!
What you’ll have
The successful candidate will have a flexible approach and able to deal with a number of enquiries and partnerships
driven from the community and external partners. You’ll be a fantastic communicator who is listens effectively
to our service users and provides clear, concise and detailed advice to a varied audience. A positive leadership
approach is key to ensuring the running of this service.

Applicants must have a full and valid driving license and vehicle to be able to travel for this role.
Qualification
Recognised professional qualification in social services and healthcare to a minimum of SVQ Level 3 or equivalent; or
to be prepared to achieve this in-role.
