You will be part of Scotland’s leading health charity providing support to people with chest, heart and stroke conditions and Long Covid to live life to the full again. Our Community Healthcare Support Service provides access to a range

of supported self management and community recovery services to support people across Scotland to manage their health as well as possible and adjust to life after a diagnosis of a chest or heart condition or after a stroke. This is an exciting opportunity to lead the establishment of a new CHSS Stroke Nurse Service within NHS Borders whilst providing strategic, operational and clinical leadership to our established Stroke Nurse Service in NHS Lothian. The postholder will play a key role in shaping and developing services, leading Stroke Nurse teams, driving quality improvement, strengthening partnerships with NHS stakeholders and ensuring the delivery of high-quality, person-centred support for people affected by stroke and TIA. This role offers the opportunity to influence stroke services across two health board areas and contribute to the delivery of CHSS's No Life Half Lived #2 strategy.

The post is split across both areas, with approximately 2 days per week dedicated to Borders and 3 days per week to Lothian. The role is primarily home-based, with occasional travel across both health board areas and other locations as required to meet service and organisational needs.

Chest Heart & Stroke Scotland is looking for an experienced and motivated Advice Line Clinical Lead Practitioner to provide strong leadership for our Clinical Advice Line Team, supporting people affected by chest, heart, stroke, Long Covid conditions and Out of Hospital Cardiac Arrest (OHCA).

This is an exciting opportunity for a registered healthcare professional to combine clinical leadership, people management and service development, ensuring high-quality, person-centred care while driving performance,

continuous improvement and delivery of organisational priorities.

About the Role:

Reporting to the Deputy Head of Service Delivery (Triage, Clinical & Support Line Operations), you will lead a team of clinical practitioners delivering telephone and video-based clinical case management and support services.

You will be responsible for clinical governance, service quality, workforce performance and achievement of service KPIs.

You will use performance data, service user feedback and quality outcomes to drive improvement and support strategic service development. The role will also provide leadership and operational support to the Stroke Nurse Team in the

absence of the Stroke Nurse Lead.

Key Responsibilities:

• Lead, develop and support the Clinical Advice Line Team.
• Deliver agreed KPIs, quality standards and service outcomes.
• Monitor performance, caseloads and service capacity.
• Provide clinical leadership, supervision and coaching.
• Lead quality assurance, audit and continuous improvement activity.
• Ensure effective risk management, safeguarding and clinical governance.
• Support recruitment, performance management and workforce planning.
• Contribute to organisational strategy and service development.
• Build effective relationships with NHS and third-sector partners.
• Support the Stroke Nurse Team during periods of leadership absence

About You:

Applicants must be resident in the UK, have the legal right to work in the UK, and possess a good understanding of Scotland's health and social care landscape. Unfortunately, Chest Heart & Stroke Scotland is unable to provide visa sponsorship.

Essential:

• UK-recognised first-level Nursing qualification with current NMC registration, or UK-recognised Allied Health Professional qualification with current HCPC registration.
• Significant post-registration clinical experience.
• Proven experience leading and managing clinical teams.
• Experience delivering services against KPIs, quality standards and organisational objectives.
• Strong understanding of clinical governance, safeguarding and quality improvement.
• Experience using performance data to inform decision-making and service improvement.
• Excellent communication, leadership and organisational skills.
• Ability to work effectively in a home-based environment.

Desirable:

• Experience supporting people affected by chest, heart, stroke or Long Covid conditions.
• Leadership or management qualification.
• Experience in service redesign, quality improvement or change management.
• Experience managing budgets and resources.

We are seeking an enthusiastic individual with a clinical qualification and registered with a UK Clinical Body (such as NMC, HCPC) and have demonstrable experience in a management role. A robust knowledge of one or more of our conditions is essential. Management/Leadership qualification desirable but not essential.

What the role will involve

The role's purpose is to provide support across CHSS’s impact and evaluation framework, helping to strengthen how the charity understands, demonstrates and improves the difference it makes for people with our conditions.

Working directly with the Impact and Evaluation Manager, the postholder will support the consistent application of evaluation practice, use of insight and outcome reporting across CHSS. The role will support teams with evaluation planning, data collection, analysis and reporting.

A core component of the role will be supporting the Service Delivery team to maintain high quality and effective insights for the Community Healthcare Support Service Model. The postholder will maintain strong engagement with the Service Delivery team and response rates from service users, ensuring that everyone who accesses our services has an opportunity to feedback on their outcomes and experiences.

About the Role

The Contracts Officer plays a key role within Service Delivery, supporting the effective management, monitoring and governance of service level agreements, statutory contracts and commissioned service arrangements across Scotland.

Working closely with managers, NHS Boards, the Scottish Government, Local Authorities and other partners, you will help ensure that services are delivered in line with contractual requirements, performance expectations and organisational priorities.

This is an excellent opportunity for someone with strong organisational skills and attention to detail who enjoys working collaboratively, managing information and supporting continuous improvement.

The role includes:

• Developing and maintaining the Service Delivery Contract Register.
• Supporting contract monitoring, review and renewal processes.
• Coordinating reports and governance information.
• Supporting quality assurance, compliance and performance monitoring activities.
• Working with managers and external partners to ensure contractual commitments are met.
• Identifying risks, concerns and opportunities for improvement.
• Supporting the ongoing development of contract management processes across Service Delivery.

About You

We are looking for an organised and proactive individual with experience in administration, governance, contract management or a similar business support role. Ideally, educated to SCQF Level 7 (or equivalent), such as an HNC, relevant vocational qualification, or equivalent professional experience gained through working in contract management, governance, administration, quality assurance, commissioning or a similar role.

You will have:

• Excellent organisational and administrative skills.
• Strong attention to detail.
• Experience maintaining records, registers or governance documentation.
• Experience preparing reports and monitoring information.
• Strong communication and relationship-building skills.
• The ability to manage competing priorities and meet deadlines.
• Good Microsoft Office skills, particularly Excel, Word and Outlook.

Experience of working within health and social care, the NHS, public sector or third sector environments would be advantageous but is not essential.
